The purpose of the Congestive Heart Failure Clinic (CHF Clinic) is to promote wellness, education and disease management utilizing advanced cardiac assessment in conjunction with the medical staff. Patients learn more about CHF, what causes it and what they can do to control it. Disease management includes close physical assessment, education and ensured compliance by assessing patients on a weekly to monthly basis.
We strive to minimize hospitalization, improve quality of life and limit disease progression by educating patients on better self-management at home.
