Crestwood Medical Center offers secure, convenient access to your personal health information through the MyHealthHome patient portal and mobile app. This tool helps you stay informed and connected before, during and after your hospital visit.

Access Your Health Information Online

Through the MyHealthHome patient portal, you can securely:

  • Securely and easily manage your healthcare online
  • View recent laboratory results
  • Obtain radiology reports 
  • View current medications
  • List current allergies
  • View immunization records
  • Review clinical summaries of the care you received at Crestwood Medical Center
  • Share your information with caregivers and other providers

Access is available anytime using a computer, tablet or mobile device

How to Create Your Account

There are two ways to enroll:

Enrollment Invitation
After your hospital visit, you may receive a text or email invitation. Follow the instructions to create your account.

Self-Enrollment
If you did not receive an invitation, you can create an account directly through the portal login page.

Download the App

You can access your account online or with one of the apps below. Once downloaded, sign in using the same credentials you use online.

Grant Access to a Caregiver or Guardian

Patients may grant secure access to a caregiver, guardian or trusted individual.

The requesting individual must first create their own MyHealthHome account. Once created, they may request access to your account. You will receive a notification with a PIN code to share. After the PIN is entered, access will be linked.

Access can be removed at any time through account settings.
