Crestwood Medical Center is committed to supporting patients before, during and after their hospital stay. Whether you’re preparing for a procedure, accessing your medical records or planning your discharge, this page provides the tools and information you need for a smooth experience.

Admissions & Registration

The Admissions team works to make your check-in process efficient and stress-free.
Where to Go

The admissions desk (Outpatient Registration) is located on the north side of the hospital, opposite the emergency room and main visitor entrance on the south side. Whitesport Drive runs between the hospital and Airport Road, as well as Whitesburg Drive, and provides access to the north side of the hospital from either route.

Be on time. During your pre-admission call with a nurse, you will be told the exact time to arrive before your procedure.

  • Insurance card(s)

  • Physician orders or prescriptions

  • Payment method for co-pays or deductibles
Payment Information
Co-payments, deductibles and patient responsibilities are due at the time of service. Insurance coverage will be verified prior to calculating your responsibility.

All patients are admitted to Crestwood Medical Center without regard to race, creed, color, sex, religion, ancestry, sexual orientation, marital status, national origin, economic status, educational background, or the source of payment for care.

During Your Stay

We strive to make your stay as comfortable as possible.

  • Non-smoking campus
Interpreters

Sign language, interpreting services and other auxiliary aids are available free of charge for hearing, visual and speech impaired patients, as well as those for whom English is not their primary language. Just ask your nurse for assistance to request any of these services.

Meals

Patients can select their meals from a seven-day cycle selective menu. Menus are distributed to patients each morning between 8 – 9 a.m. For more information, dial extension 5550 or 5828.

Breakfast: 7:45 – 8:45 a.m.
Lunch: 11:45 a.m. – 12:45 p.m.
Dinner: 4:45 – 5:45 p.m.

We also provide between-meal snacks for patients who have additional nutritional needs. Snacks are provided three times a day: 10 a.m., 3 p.m. and 8 p.m.

Going Home

When it’s time for you to leave our hospital, we want to make sure you’re prepared for a successful recovery. Take a look at the list below to make sure you’ve thought of everything, and call your doctor if you have any questions once you’ve returned home.

Discharge Checklist

Things to bring
  • Continuing care instructions from your nurse and doctor
  • Personal items
  • Items you received while at the hospital
  • Valuables you deposited in the hospital’s safe
  • Any prescriptions, a medications list and any medications being held at the pharmacy
  • Follow-up care instructions from your doctor
Things to do
  • Meet with the hospital’s discharge planner early in your stay to ensure a smooth discharge process later.
  • Visit the cashier to make payment arrangements and pay any charges not covered by your insurance.
  • Let your nurse know where you’d like packages and mail forwarded.
